Medicare Part D Temporary ID Card

After you complete your Medicare Part D enrollment, you can print a temporary member ID card. You can use this card when your Express Scripts Medicare coverage begins (as early as January 1, 2020).*

Simply print your card and then follow these steps:

Fill in your name.

Contact Express Scripts Medicare to get the membership information needed to complete your card.

Call 1.866.477.5703, 24 hours a day, 7 days a week, except Thanksgiving and Christmas. TTY users, call 1.800.716.3231.

Use your temporary member ID card each time you fill a prescription until your permanent card arrives in the mail.

*This is a temporary plan membership ID card. Your permanent ID card will be provided upon receipt of an approved application by the plan and the Centers for Medicare & Medicaid Services. You’ll receive a letter of disapproval if your application is not approved.

Express Scripts Medicare (PDP) is a prescription drug plan with a Medicare contract.
Enrollment in Express Scripts Medicare depends on contract renewal.
