Otsuka ICU Medical initiated a voluntary recall of 0.9% Sodium Chloride Injection on Sept. 17, 2026, due to a product mix-up in which bags labeled as 0.9% Sodium Chloride Injection may contain 10mEq Potassium Chloride Injection. Accidental administration of the highly concentrated potassium chloride product may cause serious adverse events, including neuromuscular dysfunction, cardiac dysrhythmias and potentially fatal cardiac arrest. No adverse events had been reported at the time of the recall announcement. 0.9% Sodium Chloride Injection, USP is indicated for fluid and electrolyte replenishment and may be used as a diluent or vehicle for compatible intravenous medications.
